Once the online referral completed with appropriate information an acknowledgement is sent via email.

Appointment booked

Contact is made within 48 hours with the client and an appointment is made. Confirmation of appointment sent to the Case Managers via email.

Assessment completed

Comprehensive assessment completed at home at scheduled time and date. Any changes to appointment will be notified to Case Manager.

Report with Recommendations sent

Home Modifications report (minor) with assistive technology links sent to Case Manager within 5 working days. If a follow up is required it will be recommended.
